By electronically signing under the Guardian Signature Agreement, I confirm that I am the legal guardian of the registered participant. I further agree that the mentioned participant is the age of ten or older.
I am aware of all of the risks of participating in any/all activities associated with this event. Participation in the aforementioned activities may expose the participant to certain foreseeable and unforeseeable risks of damage and/or bodily injury, including serious bodily injury, where the participant may need to be hospitalized.
I knowingly, freely, and voluntarily assume all risks and the participant is knowingly, freely, and voluntarily engaging in the above-mentioned activities.
I hereby release Porter County Public Library System (PCPLS), and/or their directors, officers, employees, volunteers, representatives, and agents, and the activity holders, sponsors, and volunteers, from any and all liability arising out of the participant’s participation of the above-mentioned activities and hereby waive our rights herein to assert any claim(s) for damages, bodily injury or serious bodily injury to the fullest extent allowed by law.
I further agree that the participant and I will hold harmless PCPLS, and/or their directors, officers, employees, volunteers, representatives, and agents, and the activity holders, sponsors, and volunteers, against any and all claim(s) for damages, bodily injury or serious bodily injury arising out of or in connection of the participant’s participation in the above-mentioned activities whether caused by negligence or otherwise.
Kouts Public Library has been a fixture in the town of Kouts for over 40 years. In 1995, our current building was constructed, anchoring the south end of town. We offer a wealth of amenities to the citizens of Kouts and neighboring townships, including programming for youth and adults, meeting room spaces, wi-fi, and computers. The Indiana History Room contains a collection of artifacts and documents related to the town's history and its founding families. Our collections of materials have grown from books and periodicals to include movies, audiobooks, ebooks, music, streaming videos, and more.
